Before You Arrive

Save yourself time on your first visit by completing your new patient paperwork ahead of time. You can download and fill out your forms at home, at your own pace — no rushing in the waiting room.

Print, complete, and bring them with you — or ask our team to email them to you directly.

Don’t have a printer? No problem — arrive 20 minutes early and we’ll have everything ready for you at the front desk.

 

What to Bring on Your First Visit

We’ll take care of the rest — just bring these four things and you’re set:

1 A photo ID Any government-issued ID works perfectly.
2 Your insurance card We’ll verify your benefits and walk you through your coverage — no guesswork on your end.

4 Your copayment (if applicable) We accept most major payment methods. If cost is a concern, please ask us — we’re here to help you find a path forward.

 

What to Expect at Your First Appointment

Your initial evaluation is the foundation of everything we’ll do together. This isn’t a rushed 15-minute appointment. We take the time to truly understand what’s happening in your body — where the pain is coming from, how it’s affecting your daily life, and what a whole-body, root-cause approach looks like for you specifically.

Come dressed comfortably in clothing that allows us to assess your movement freely. And come with questions — we love them.

Your initial evaluation is typically 60–90 minutes. We want to understand the full picture — not just the place that hurts.

 

Comfortable, flexible clothing you can move in freely. If we’re evaluating your lower back or hips, athletic shorts or leggings are ideal.
